Early Intervention Process
1. Parents fill out the enrollment interest form below.
2. Cargivers/the person filling out the form will receive a call from Green Mountain Sprouts to begin the enrollment process. An intake packet will be emailed /mailed to the family to complete prior to setting up the initial meeting.
3. A board certified behavior analyst (BCBA) will meet with the family and child, go over any additional paperwork, and begin the initial assessment. This may take more than one session.
4. Based on the evaluation/assessment results the BCBA will prescribe programming recommendations and create an individualized treatment plan that will be shared with the family.
5. Once the family approves the treatment plan recommendations the BCBA will submit all paperwork to the child's insurance agency for authorization. Once authorization is approved with the insurance agency the BCBA will contact the family and set up a start date at the clinic.
